Introduction
I wish all who celebrate a very happy Easter next week. I will be taking a break over the Easter period but will be back the weekend of 27 April.
I rarely comment on specific links, but if I could encourage attention to the ‘Responding to Harm’ report published by Patient Experience Library, it would be gold dust for those of us looking at how poor organisational culture and behaviours lead to harmful practices and how to identify this. I will be using it at work with my team.
